The following appeared on 9 August 2002 in The Oregonian: Dr. Thomas Dewitt Forner died Aug. 4, 2002, at age 49.
Dr. Forner was born Aug. 4, 1953, in Houston. He attended medical school at the University of Texas and the Free University of Brussels. He moved to Portland in 1985 and completed his residency at Providence St. Vincent Medical Center. He practiced with Family Care Inc. In 1978, he married Diane Pace.
Survivors include his wife; daughter, Robin; sisters, Janet Colonna and Barbara Forner; and brothers, Michael and Mark.
